Existing Patients-Membership FAQ's
We are excited to announce that Dr. Worden will be offering a limited number of memberships starting July 1, 2023 to her current patients and open her practice up to new patient members. These memberships will provide access to frequent and longer visits with Dr. Worden, urgent appointments, more access to her in between appointments for questions and concerns as well as access to online educational content. Membership options will be ideal for those working towards identifying and treating the root cause of chronic illness and supporting you on your journey towards long term health and wellness. These BFM Memberships will be limited, so we encourage you to sign up soon before they are filled. Once the limited membership spots are filled, we will place interested patients on a waiting list.
In order to move closer to a model that will help in root cause medicine, working towards optimal health and wellness for her patients, Dr. Worden will be moving towards a full “hybrid” membership model over the next year. For now, those patients not choosing a membership plan will continue to be able to see Dr. Worden using the current fee-for-service (FFS) model utilizing insurance for those with in network coverage and cash pay for those out of network.
Our goal, as always, is to provide the best care possible for our patients while allowing Dr. Worden to also take good care of herself, attend the many cutting-edge conferences and have time to develop more resources to support our patients.
We hope that this may be a beneficial change for many of our patients and support your journey to wellness. While memberships are currently optional, there will be some changes to visit scheduling after July 1st, 2023. Please see FAQ below for link for changes.
BFM Membership FAQ
What do the membership fees cover?
The membership fees are for services outside of standard office visits. Please see this list of membership benefits for a complete list of what is included with each membership tier.
Will the membership fee be used to pay for office visits?
No, we will continue to bill insurance or cash pay if out of network for office visits. This includes in person office visits, televisits, e-visits and group visits. Your patient responsibility will still be due based on contracts with your insurance company or our cash pay fee schedule if you are out of network.
How do I sign up for a membership?
If you'd like to sign up for a membership, just let our staff know and we will get you the necessary consent form to sign and make sure we have an up-to-date credit card on file.
How do I pay for monthly membership?
Memberships will be charged using your credit card on file automatically once a month. If you choose to prepay, you may prepay with either a credit card, check or cash. If you choose to not have a card on file, memberships must be prepaid with cash or check.
Are there discounts if I want to prepay for the membership?
If you choose to prepay your membership fees, there are discounts available. If you pay for 3 months in advance, there is a 10% discount. If you prepay for 6 a month membership, there is a 20% discount.
How long are the membership terms?
Memberships are for a minimum of 3 months. You can upgrade your membership to a higher tier at any time but you must be at that tier for at least 3 months before you cancel or downgrade. After the initial 3 month term, you can extend on a month by month basis.
What if I am an existing patient and do not want to choose a membership plan?
For current existing patients prior to July 1, 2023, If you do not want to choose a membership plan, there will be some changes to the scheduling policies. Please see this PDF for more information related to existing patients not choosing a membership.

Is membership mandatory?
Current existing patients do not need to become a member but given the benefits, you may choose to take advantage of one of the membership options.
New patients- yes, we do require a membership option to be chosen. After your first appointment with Dr. Worden, if you choose to continue working with her, then you can decide which membership option works best for you. This will provide in-depth time needed for the functional medicine approach as there is often close follow up regarding treatment plans, lab testing and our goal is to support you in what often is a new process. Also, many of the member benefits will be of considerable benefit to you as you are understanding and making lifestyle changes.
Can new patients become a member before seeing Dr. Worden for the first time?
We encourage new patients to meet with Dr. Worden for your first new patient visit before choosing a membership. This will allow you to decide if working with Dr. Worden is truly what you want to do, before you spend any money on a membership you may choose not to utilize.
What happens if I can no longer see Dr. Worden?
In the event that you will not be able to continue care under Dr. Worden due to extenuating circumstances such as moving to a state in which Dr. Worden is not licensed or insurance change that would not allow coverage for visits with Dr. Worden, please discuss this directly with our membership coordinator. We are willing to be flexible and accommodate such circumstances.
What if I want to cancel my membership?
If you would like to cancel your membership, just message our office and we will cancel the membership and also the recurrent charge on the credit card. We do ask for a minimum 3 month term and any cancellation prior to that will not be refunded. This cancellation must be made in writing, so a portal message, text or email will suffice. Please note, once the membership is canceled, you will no longer have access to the membership benefits. When you are ready to restart, you can always restart the membership at any time. There are no refunds of any pre-paid membership fees.
If I cancel a membership, will I still have access to purchase supplements online?
Yes, all patients who have registered with Fullscript, Pure Caps Pro or have the link to order from other supplement companies, will continue to have access to order, what will change however is the discount offered on Fullscript.
What happens to the iCARE device if I cancel membership?
If you have chosen tier 3 and have been an iCARE device rental paid by membership dues, then you can either choose to return the device at the termination of your membership or continue to rent to own the device.
