What if I do not see my insurance plan listed?

It is your responsibility to determine if your insurance is contracted with our providers.

If insurance is not contracted then visit will be out of network and you will need to pay cash for your visit.  You will be asked to pay for visit at time of service. You can submit the super-bill to your insurance and they may or may not reimburse you based on your plan as some have out of network benefits.

​We are a micro-practice which means that in order to take limited payments from insurance, spend adequate time in a visit, we do not have staff to check insurance coverage. This is your responsibility.  If you are unsure, please have a family or friend help you.  You can contact your insurance and give them the information below for the providers at Bloom Functional Medicine, Riverside Family Practice or Keith Kale, DO.
