It is your responsibility to determine if your insurance is contracted with Bloom Functional Medicine, Dr. Worden, Dr. Meigs or Dr. Bowman. If insurance is not contracted then visit will be covered as an out of network provider. For some insurances, this means it will pay nothing. You will be responsible for the cost of the visit.
Bloom Functional Medicine is a micro-practice which means that in order to take limited payments from insurance, spend adequate time in a visit, we do not have staff to check insurance coverage. This is your responsibility. If you are unsure, please have a family or friend help you.
