Televisits are available to all patients but patient must be located in Oregon (Dr. Kale) or Oregon/Washington (Dr. Worden & Meigs only) due to state licensure restrictions.
Please note that not all insurance companies will pay for televisits so please review with your insurance company prior.

Please be sure to include your name, preferred way to communicate with you and which provider you are interested in seeing.
Note each provider has their own new patient coordinator and we need to know which specific physician you are interested in seeing. Please see our provider page to learn more about each provider.

Our team will then send you the required new patient forms to get you started. ​Once new patient forms and applicable medical records are received, the doctor will review and then our staff will contact you to schedule a new patient appointment.
This process often takes 1-2 weeks but can take longer for functional medicine visits.

For patients interested in seeing Dr. Worden for functional medicine, please see the information regarding the membership program before choosing to continue. Her panel for functional medicine is currently closed but she is taking patients for her wait list. Wait list patients will be accepted as spots open up on her panel. Her current wait list is 12-24 months long. If you are needing urgent care, please ask your primary care provider to send a referral and cases will be reviewed on a case by case basis to see if exceptions can be made.
