Schedule an Appointment

Current patients:  Please call or text the office to schedule an appointment. â€‹

Televisits are available to all patients but patient must be located in Oregon (Dr. Kale) or Oregon/Washington (Dr. Worden & Meigs only) due to state licensure restrictions.

New Patients:

If you would like to become a new patient, you will need to complete new patients forms and return them before you can schedule your first appointment.  Our new patient coordinator will help guide you through the new patient process.

Once new patient forms and applicable medical records are received, the doctor will review then our staff will contact you to schedule a new patient appointment.  This process often takes 1-2 weeks but can take longer for functional medicine visits.

For patients interested in seeing Dr. Worden for functional medicine, please see the information regarding the membership program as she is currently only accepting new patients through the BFM Membership program.
